Forms are best viewed by using a current version Adobe Acrobat Reader. You can download it for free.
Forms used by the Boise State Financial Aid Office are to be printed, signed, and then submitted by mail or fax, in person, or as a scanned attachment to an e-mail message sent to FAQuest@boisestate.edu.
Please note that some forms are to be submitted only when you have a Financial Aid To Do item requesting the form. Other forms are used when you have a request or an appeal.
When attempting to locate a form that the Financial Aid Office is requesting, print only the form listed on your To Do instructions or in a BroncoMail email message. For example: when initially submitting verification documents, a student will print out either the Verification Form for Dependent Students (if your parents' information was included on the FAFSA) OR the Verification Form for Independent Students. The remaining forms may be requested later if further clarification is needed while verifying the application.
